Hospital-Bag Checklist
A starting checklist for packing quickly - written for families who may need to leave in a hurry, including anything your child depends on every day.
This is a general starting list, not medical advice - add or remove whatever fits your family.

Documents to bring
- Insurance card(s)
- Photo ID for yourself
- Your child's ID or birth certificate copy
- A current medication list (names, doses, and times)
- A current care plan or summary, if you have one
- Emergency contact list
- Doctor and specialist phone numbers
Medications and equipment
- Current medications, in their original containers
- Any medical equipment or mobility aids your child uses regularly
- Extra supplies, chargers, or batteries for that equipment
- A small bag or pouch to keep medications organized
- A backup of anything that would be hard to replace quickly
Comfort items for your child
- A favorite toy, blanket, or stuffed animal
- A comfort item your child depends on to feel calm
- A tablet, headphones, or a favorite show downloaded for waiting
- A change of clothes for your child
- A small pillow, if your child has one they prefer
Personal care
- Diapers, pull-ups, or toileting supplies, if needed
- Wipes
- Toothbrush and toothpaste
- Snacks and a water bottle (check with staff about any restrictions)
- A comb or hairbrush
For you, the caregiver
- Phone charger
- A change of clothes
- Snacks and water
- Cash or a card for parking, food, or vending machines
- A notebook and pen for writing down instructions
- A pillow or light blanket, in case you're staying overnight
Before you go
- Who to call or notify
- A plan for other children or pets at home
- Work or school notified, if needed
- Your care team or family's contact info shared with someone at home
